The following are the rules and procedures that I will be using in my classroom this year. Please take a few minutes and carefully read over this form, sign it and return with your son/daughter. If you have any questions feel free to contact me. I will be more than willing to assist you in any way I can. I am looking forward to working with your son/daughter this year and I know that if we work together he/she can receive the quality education that he/she deserves. However, it is of the utmost importance that we establish the lines of communication early. So, once again if you have any questions now or in the future do not hesitate to contact me.

REQUIRED MATERIALS: (Please bring each day)
- Writing utensil (pencil recommended)
- Calculator (you may check one out of the library)
- Spiral or Composition notebook (1 subject)
- Paper
- Homework as assigned
v Textbooks, if needed, will be provided in class
ATTENDANCE: Attendance is mandatory. Absence from a class can seriously hamper your success in this class (especially on the block schedule). If you are absent from a class it is your responsibility to obtain the make-up work, missed notes, tests, and quizzes. You will be allowed one day plus one extra day to complete the missed assignments. With the block schedule it is extremely important that you obtain your make-up work as soon as possible (meaning if you miss Monday, it is your job to get the missed work on Tuesday). The longer you wait, the more material you will miss. This must be done on your time, not the instructor’s. The “buddy system” is suggested.
TUTORIALS: Tutorials will be available Monday - Friday, 7:30am-8:00am
Tuesday, 3:45pm-4:15pm
NOTES, WARM-UPS, AND SUMMARIES: Students will be required to take notes throughout the course. Warm-ups will be given at the beginning of each class. Summaries will be completed at the end of class. These exercises are to be kept in a spiral.
HOMEWORK: Fluency in mathematics requires practice. Homework assignments will be giventhroughout the six weeks as practice. Homework quizzes will be an evaluation counted in the student’s daily grade. Homework quizzes will be given at the beginning of the week. Completion of the homework is important because the student is allowed to use these on their homework quizzes. Make-up quizzes are allowed if absent. If a student fails a homework quiz they are allowed to retake ONE QUIZ PER SIX WEEKS. Retake is possible provided all homework is completed and student attends one tutorial session outside of class to review material with the instructor.
HOMEWORK GUIDELINES:
- All problems should be worked out in full.
- Label each page of homework completed with your name and class period in the upper right hand corner.
WARM-UP QUIZZES: After a question and answer session, a short quiz will be given at the beginning of class over the previous homework given. (This will occur several times throughout the six weeks.)
TESTS: Students will have the entire class period to complete tests (unless otherwise announced). If a test is missed, make-up tests will be given at the Learning Center or during tutorials. If it is necessary for you to miss a test because of a school activity, you must arrange to take the test either before you leave or immediately upon return. Tests must be completed within 1 week of your return to class. We will have at least 2 tests each six weeks. You may “retake” one exam, per six weeks after completing the following:
- Correcting the previous test
- Attendance at one tutorial discussing your corrections
ASSESSMENT OF OUTCOMES:
- Tests
- Homework Quizzes (Daily grade)
- Warm-up Quizzes (Math Lab grade)
GRADING SYSTEM:
- Students will be evaluated on their performance on tests, quizzes, homework assignments, Math Lab, and other daily assignments.
- The following is a breakdown of the student’s grading system:
Tests – 50% A = 90% +
Daily – 25% ** B = 80% +
Math Lab – 25% C = 70% +
Below 70% failing
** At least one grade will be dropped from the daily scores (lowest grade).
CLASSROOM EXPECTATIONS:
Students are expected to:
- When the bell rings:
- Sit down
- Face the front
- Quietly complete your warm-up
- Put your backpack/purse on the floor and place these items in plain sight on your desk:
- Pencil or Pen
- Notebook or Paper
- Calculator
- Warm-up
- Previous Homework Assignment (if assigned)
- Make quick and smooth transitions
- Respect yourself, others, and the classroom
- Do not distract others
- No cheating on class work will be tolerated
- Do not leave litter
- All cell phones must be turned OFF before you enter the classroom
- No uninvited talking
Failure to meet these expectations will result in the following consequences:
First offense – warning
Second offense – 10 minute detention
Third offense – 30 minute detention
Failure to serve detention will result with an office referral
TARDY POLICY: A student will receive an office referral upon every fifth tardy of each six weeks. This referral will be issued by the student’s assistant principal. This is in accordance with the Weatherford High School tardy policy. A student must be in class before the tardy bell rings or he/she will be counted tardy.
